The individual or group primarily responsible for each major task in the organization should be agreed on by a governing body. This individual or group is responsible to complete all activities related to the task. Likewise, the leaders that have accountability for successful execution should be identified for each task. In some cases, multiple responsible or accountable parties may be recognized. However, in many cases, accountability is strengthened when a single responsible party and a single accountable party are acknowledged. The task governance chart can be used to identify the responsible and accountable parties for a given task.
