Purpose

A guide and template for leading regular manager-employee conversations regarding performance over a short (e.g., 1-4 weeks) period of time. The form is designed to guide conversations that occur with regularity as opposed to annually, adhering to the principle of frequent but brief discussions that allow the early identification of issues hindering performance and coaching to resolve such issues and challenges. Sample questions should be adapted as appropriate for use with different levels of employees.
 

How to use this tool

The functional manager utilizes the tool as a guide to ask questions that draw out the status of current work efforts and any challenges the employee is facing that may require either guidance or intervention from the manager to resolve.
