Purpose

The Process Activity and Sequence Worksheet provides a structured method for documenting how a task, process, or workflow is performed from beginning to end. It helps teams establish clear process boundaries, identify every activity required to achieve the intended outcome, place those activities in the correct sequence, and capture the roles and technologies involved.

The completed worksheet creates a shared understanding of the current process and provides a practical foundation for identifying information exchanges, dependencies, decision points, handoffs, inefficiencies, and improvement opportunities. It can also serve as the source information for developing a process flowchart or conducting more detailed process analysis.

How to use this tool

  1. Select the process or workflow: Choose a defined task or workflow that is important to organizational performance, involves multiple activities or participants, or may benefit from greater clarity or improvement.
  2. Establish the process boundaries: Define the event that initiates the process, the outcome that indicates completion, and the activities included and excluded from the review.
  3. Gather input from those performing the work: Engage employees, managers, support functions, and external parties with direct knowledge of how the process is completed. Input can be collected through surveys, interviews, observation, or facilitated working sessions.
  4. Document each activity: Record every activity required to complete the process, including activities performed by individuals, teams, support functions, technologies, suppliers, contractors, or other external parties.
  5. Place activities in sequence: Number the activities in the order in which they occur. Identify the preceding activity or trigger to clarify where each step fits and note where an activity may return to an earlier step.
  6. Capture current work practices: Identify the primary performer, systems or tools used, relevant process variations, and established practices that affect how the activity is completed.
  7. Review and validate the sequence: Ask participants to confirm that the documented process reflects how the work is actually performed, not only how formal procedures state that it should be performed.
  8. Use the completed worksheet for subsequent analysis: Apply the documented activities and sequence when identifying dependencies, information requirements, decision points, handoffs, improvement opportunities, or automation potential and when creating the process flowchart.
